Only one laundry dispenser had solid bleach available in the dispenser. The one that had bleach did not appear to be working properly. The other two did not have disinfectant available. Repair dispensers. Call 919-989-5194 when repaired. Must be washed in at least 50ppm chlorine.
The carpet in the resident rooms, hallways and common areas is stained and in bad repair. Suggest replacing all damaged and carpet in the facility.

Shower curtains in resident private restrooms has a pink/black build up on the bottom. Clean or replace all shower curtains that are in bad repair. In the common shower on the Clair Bridge unit, joint between the shower and floor is moldy and needs cleaning. Shower seats have a small amount of mold on the under side of units. Clean to remove and prevent.
Several sinks spread out around facility were 117-120 degrees. Make sure water temps are between 110-116 degrees in all areas that residents have access to.

One machine had the feed lines reversed and the machine in the memory unit would not pull sanitizer from the bottle to the machine. Do not use the machine in the memory unit until repaired.
Filters for the ventilaiton units in several rooms on Clare Bridge Hall and Wing by office need better cleaning to remove and prevent a heavy build up. In a few rooms, the HVAC units are separating from the wall due to possible water leak in structure, repair. Clean the units with a mold/mildew on vents in these rooms and rec. room.
