Throughout the facility: walls are showing wear from contact with walkers/wheel chairs. The corners are damaged in resident rooms from contact with walkers/chairs. Repair all damaged walls and corners throughout the facility. Suggest installing corner protectors and a more durable material (FRP, plastic sheets, etc.) on the walls behind beds and in the hall ways in the affected areas.

Mattresses on resident beds are cracking which is exposing the inside foam material. Once the cover is cracked they are no longer moisture resistant and not in good repair. Replace all mattresses that are in bad repair so they are water resistant and able to be easily disinfected.

In the nurses station the fridge that is used to hold the medications and snacks/drinks for the patients was left open and so air temp. in the unit was at 60F. There was no food in the fridge at this time. Make sure to fully shut coolers and that it is at 45F or below for all cold hold of potentially hazardous foods.

Hot water in the assisted living halls was at 117-118F. Hot water in the dementia halls in some rooms got up to 117F. All how water should be at 100-116F. Maintenance is turning down the water heater now.

One of the medicine carts in the dementia hall was not locked. When employee tried to lock it the cart would not lock. Another employee was able to make it lock. Later in the inspection when walking by checked the cart again and it was unlocked. Make sure to keep medicine carts locked. Repair cart that is coming unlocked. Employee relocked the cart and stated she would keep a eye on it to make sure it stays locked until it is repaired.

In a few of the resident rooms, walls behind the beds are starting to show wear and slight damage. Suggest finding a barrier to place between bed and wall to prevent damage, or install a wall cover that will protect the sheet rock from damage (FRP, Plastic, etc.).
