On the 6th grade hall boys bathroom a sink was missing both nobs. In the boys locker room a shower head was missing. All fixtures shall be in good repair.

Trailer 13 had holes in the walls. Repair all holes in the walls, Walls floors and ceilings shall be in good repair.
Trailer 621 has a damaged section on the wall and carpet, door is not secure at the top which is allowing rain water in. Trailer 623 has very dirty carpet, there are ceiling tiles that show signs of water damaged, and the floor on sides of the entrance door is crunchy (may be water damaged). Trailer 822 has holes in the wall. Repair all holes in the walls, damaged walls, clean carpets where needed and replace all damaged spots. Replace all water stained ceiling tiles.
There is a large hole in the wall in one of the mobile units. In another mobile unit the carpet is in bad repair in some areas. PIC stated that there are work orders already for both of these. There was one ceiling tile in the hallway between the locker rooms that was broken. Replace.

A few dead insects seen through out the facility. Continue pest control. Make sure the premises is kept neat and clean to prevent pest breeding and harborage.
Carpets in a few of the classrooms on all hallways and the mobile classrooms showing signs of wear and stains. If unable to clean then replacement is recommended. Repaint all walls in classrooms that are scuffed.

Severely peeling paint on the ceiling of the gym. Scrape and repaint ceiling. Floor cracking in the 7th to 8th grade corridor. Cracks in the wall in the girls and boys locker rooms. Cracks in cinderblock walls in rooms 613, 617 and 618. Damage in back wall of trailer unit 619. Torn carpet and buckled carpet in trailer unit 621. Stains in carpets in several rooms. Damage to front wall in trailer unit 619. Clean the floors behind the bleachers in the gym.

Floors in classrooms with tile floors need cleaning. Carpet torn in trailer #821. Clean gym floor behind stands of dust. A number of ceiling tiles in the media center are water stained. Repair if roof is still leaking and replace the stained tiles.

General Comments
Return air vents in some of the classrooms need cleaning of dust. Several water fountains have cut ons that will not work. Water flow on one water fountain in the 6th grade hall needs to be increased to go beyond the rim.
Detail clean all water fountains on a regular basis to prevent growth.

General Comments
If potentially hazardous foods (pizza, hot dogs, hamburgers, etc.) are going to be sold from concession stands, the concession stand must recieve a Limited Food Service Food Permit from this office prior to selling food. These permits expire on Dec 31 of the year it was written and must be reissued at the beginning of each season. Can drinks, candy, popcorn, pretzels, cotton candy do not need a permit to sell.
