General Comments
This permit is issued to this artist at a temporary set up location in the Bulldog Harley Davidson Store. The hand sink in the breakroom is set apart from all other use and can only be used by the artist on the day of the event. Breakroom does not have a door and there is a clear line between the tattoo location and the handsink which is about 20 feet away. The area that the artist are set up has tables and barrier rope set up around it to confine the artist set up area and restrict the public from proximity. Trash will be disposed of on site in dumpster and the sharps container with waste will be returned to the shop for disposal. This permit is valid for 1 year to this artist at this exact set up location the way it was inspected and permitted today. Notify this office if you are going to perform tattooing at this set up location over the next year. This set up cannot be altered and the permit is tied to this set up.
